NEW YORK CITY (November 26, 2012) — It is EmblemHealth policy, and federal law, to protect your privacy. We do not share information about you with anyone else unless permitted or required by law or you sign an authorization giving us permission to do so.
This policy applies to all of our members, regardless of race, gender, age and sexual orientation, as mandated by the Health Insurance Portability and Accountability Act of 1996.
EmblemHealth members must complete the Authorization to Use and Disclose Protected Health Information form in order to share personal health information. The form can be downloaded here.
