EmblemHealth Essential Plan (with Adult Dental and Vision)

Available year-round through the New York State of Health Marketplace.

A plan for New Yorkers with a $0 monthly premium, no deductible, and no referrals required to see a specialist. These plans also allow members to see a primary care physician, mental health professional, or dermatologist virtually with our Teladoc P360 benefit at no extra cost. Eligibility is based on income and other factors.

Medicaid Managed Care

If you’re eligible for Medicaid, then you may qualify for one of EmblemHealth’s state-sponsored Medicaid Managed Care plans, which offer low-or no-cost health, vision and dental coverage.

Deductible: $0 individual / $0 family

Enhanced Care Plus (HARP)

Enhanced Care Plus is our state-sponsored Health and Recovery Plan, or HARP. A HARP plan provides Medicaid members with additional health care resources to help members take care of all their physical health, behavioral health, and  non-Medicaid support needs. There is no monthly premium payment for this plan.  Members get access to excellent doctors within the EmblemHealth network.

Child Health Plus (CHPlus)

There are no copayments for any CHPlus benefits, but you may have a monthly payment depending on your family's monthly income and your family size compared to the most current Federal Poverty Level (FPL) guidelines.

Low or no monthly costs.

Individual and Family Plans Serviced by Our Millennium Network

These plans offer no-cost preventive care, prescription drug coverage, convenient telemedicine, pediatric dental and vision, and more. If you live in the five boroughs of New York City, Long Island, Westchester or Rockland counties, your provider network will be our Millennium network.

You may qualify for financial help based on your income.

Individual and Family Plans Serviced by Our Select Care Network

These plans offer no-cost preventive care, prescription drug coverage, convenient telemedicine, pediatric dental and vision, and more. If you live in Albany, Broome, Columbia, Delaware, Dutchess, Fulton, Greene, Montgomery, Orange, Otsego, Putnam, Rensselaer, Saratoga, Schenectady, Schoharie, Sullivan, Ulster, Warren, or Washington counties, your provider network will be our Select Care network.
