EmblemHealth Essential Plan (with Adult Dental and Vision)

Available year-round through the New York State of Health Marketplace.

A plan for New Yorkers with a $0 monthly premium,no deductible, and no referrals required to see a specialist. These plans also allow members to see a primary care physician, mental health professional, or dermatologist virtually with our Teladoc P360 benefit at no extra cost. Eligibility is based on income and other factors.

Medicaid Managed Care

If you’re eligible for Medicaid, then you may qualify for one of EmblemHealth’s state-sponsored Medicaid Managed Care plans, which offer low-or no-cost health, vision and dental coverage.

Deductible: $0 individual / $0 family

Enhanced Care Plus (HARP)

Enhanced Care Plus is our state-sponsored Health and Recovery Plan, or HARP. A HARP plan provides Medicaid members with additional health care resources to help members take care of all their physical health, behavioral health, and  non-Medicaid support needs. There is no monthly premium payment for this plan.  Members get access to excellent doctors within the EmblemHealth network.

Deductible: $0 individual / $0 family

Child Health Plus (CHPlus)

There are no copayments for any CHPlus benefits, but you may have a monthly payment depending on your family's monthly income and your family size compared to the most current Federal Poverty Level (FPL) guidelines.

Low or no monthly costs.

EmblemHealth Millennium Platinum

This is a good plan if you receive frequent care because the deductible is $0. It also includes no-cost preventive care, convenient telemedicine, prescription drug coverage, and more. This Platinum plan uses the Millennium network and is available within nine counties.

In-Network Deductible: $0 individual / $0 family 

EmblemHealth Millennium Gold

This plan is a good choice if you visit the doctor often but want to pay less than you’d pay for our Platinum plan. It has a low deductible and includes no-cost preventive care and convenient telemedicine. Plus, prescription drugs are covered before the deductible is met. This Gold plan uses the Millennium network and is available within nine counties.

In-Network Deductible: $600 individual / $1,200 family 

EmblemHealth Millennium Silver

This is a good plan if you go to the doctor for regular visits. It has a low premium and includes no-cost preventive care and convenient telemedicine. Plus, one primary care visit and prescriptions drugs are covered before the deductible is met. This Silver plan uses the Millennium network and is available within nine counties.

In-Network Deductible: $2,100 individual / $4,200 family 

EmblemHealth Millennium Bronze

This is a good plan if you don’t seek frequent medical care and would like to save some money on your health insurance. It has the lowest premium of our metal plans and includes no-cost preventive care and convenient telemedicine. Plus, three primary care visits are covered before the deductible is met. This Bronze plan uses the Millennium network and is available within nine counties.

In-Network Deductible: $3,800 individual / $7,600 family 

EmblemHealth Millennium Catastrophic

This plan is for individuals under age 30 and others who qualify based on financial need. It includes three no-cost primary care visits. This Catastrophic plan uses the  Millennium network and is available within nine counties.

In-Network Deductible: $9,200 individual / $18,400 family

EmblemHealth Select Care Platinum

This is a good plan if you receive frequent care because the deductible is $0. It also includes no-cost preventive care, convenient telemedicine, prescription drug coverage, and more. This Platinum plan uses the Select Care network and is available within nineteen counties.
