Special Needs Plan Model of Care Provider Training

Each year, all Medicare providers are required to complete the Special Needs Plan (SNP) Model of Care (MOC) training for each of the dual-eligible SNPs in which they participate, as mandated by the Centers for Medicare & Medicaid Services (CMS). Providers in the VIP Bold and Reserve Networks must complete the combined EmblemHealth and ConnectiCare SNP MOC provider training.

To satisfy this requirement, providers must complete the 2024 EmblemHealth and ConnectiCare SNP MOC training and submit the applicable attestation below by Oct. 1, 2024.

If you are an individual provider, click Individual Attestation. If you are attesting on behalf of a group, click Group Attestation. A group is more than one provider who share a valid tax identification number (TIN). You must enter a valid group TIN for the providers in the group to receive credit for completing the training. A separate attestation is required for each group/organization TIN. If you are an individual provider or a group that participates with both EmblemHealth and ConnectiCare, your attestation applies for both companies.
