Whether you’re looking to prevent or manage diabetes, the Community Diabetes Wellness Program* at EmblemHealth Neighborhood Care is here to help you make a meaningful difference to your overall health. Diabetes is one of the most common chronic conditions in the United States. But with the right guidance and support, you can live a healthy life. This free program provides screenings, educational courses, and wellness activities that can help you make small but consistent changes to your lifestyle to help keep your blood sugar under control.

To join this program, visit a participating Neighborhood Care location for a diabetes screening event, or call for more information.

No. Classes and A1C tests are all completely free. You do not need a referral from your doctor to participate. 

Yes! Classes and screenings are open to the community. 

An A1C test measures average blood glucose over the past two to three months. It can identify prediabetes or be used to diagnose diabetes. An A1C test can also monitor how well you are meeting your treatment goals.

Diabetes education can be helpful when:

  • You have recently been diagnosed with prediabetes or diabetes.
  • You’re having trouble managing prediabetes or diabetes.
  • Your medicine has changed, or you have a new treatment plan.
