Electronic Notice Option for Medicaid and HARP

EmblemHealth and our health care partners can now send you electronic notices about service authorizations, plan appeals, complaints, and complaint appeals rather than sending the notices by phone or mail.

You can also choose someone like a family member, friend, or lawyer to act on your behalf. The person you choose can get copies of your notices electronically. If you want someone to act on your behalf, please complete this form:

Designee Authorization To Use or Disclose Protected Health Information (PHI) and the Member Portal

This form should be used if you want to allow someone else to view your health information in the portal and make certain changes to your account.  

If I choose to receive electronic notices, how will they be sent?

EmblemHealth and our health care partners will send these notices to you through our member portal, myEmblemHealth. Some of our health care partners (those listed below) will send them to you through their portals. You will need to register separately for their portals.
