NEW YORK, NY— EmblemHealth, one of the nation’s largest nonprofit health insurers, launched two brand-new membership resources designed to build on its commitment to better health outcomes and member experiences. The Hub, an outbound engagement team, meets members where they are. All EmblemHealth and ConnectiCare members can now register for brand new personalized portals, a digital platform and mobile app enabling them to access their health care journey right at their fingertips.

The Hub provides a more seamless end-to-end journey for EmblemHealth members where teams provide comprehensive outreach unique to the member’s needs. Members receive fewer and more impactful outreach addressing multiple needs in a single interaction—from conducting welcome calls to completing health risk assessments (HRAs), addressing gaps in care, scheduling a visit to a PCP, and more.
