If you are enrolling in a health plan on the exchange marketplace, you’ll need to go to nystateofhealth.ny.gov site to enroll.
You will need to have certain information with you. Before applying, gather these items for each member of your household who needs health care coverage:
- Social Security numbers or document numbers for legal immigrants.
- Employer and income information for example, from your pay stubs or W-2 forms – Wage and Tax Statements.
- Policy numbers for any current health insurance plans covering members of your household
- Email address required to establish account.
After you enroll, contact the plan to make your first premium payment.
You must pay your initial plan premium by the 10th of the month your enrollment becomes effective in order for your coverage to begin. For example, if you enroll by December 19 for January 1 coverage, you must pay your first premium by January 10. If you don’t make your first payment by January 10, you will not be considered a member of the plan and any services you receive will not be covered.
