- Part C Medical Explanation of Benefits (EOB) – Mailed to you every three months and helps you track your out-of-pocket cost, the amount you will have to pay for health services, for each quarter and year-to-date. Part C includes Medicare Part A (hospital) and Part B (medical).
- Claim Explanation of Benefits (EOB) – Mailed to you each time a claim is submitted and explains what we covered and the status of your claim.
