EmblemHealth Partners with Cityblock Health to Bring New Care Model to New York

We are excited to announce our new partnership with Cityblock Health, a company that leverages technology and a high-touch, personalized approach to care to provide medical, behavioral and social services to Medicare and Medicaid patients in urban, underserved communities.

Cityblock Health is a startup from Alphabet’s Sidewalk Labs, which imagines, designs, tests, and builds urban innovations to help cities meet their biggest challenges.

So what does that mean for you, our members? In short, a more comprehensive and integrated health care experience.

Cityblock Health will identify members with complex needs – beginning in Crown Heights, Brooklyn – and will assign them to a care team of doctors, nurses, behavioral health clinicians, and community health partners who will create action plans to provide continuous support. Cityblock Health’s digital platform, Commons, will facilitate  seamless communication between the care team and our members.

The care and services provided through this partnership are unique in that they can happen when and where is most convenient for the member. But we also recognize the importance of having a place where health and community merge. So this summer, the first Neighborhood Hub will open at the co-located EmblemHealth and AdvantageCare Physicians office in Crown Heights, Brooklyn.

We look forward to growing this partnership, which we hope to expand to more members across the New York area.
