New Post-Acute Care Process Dec. 1, 2022
EmblemHealth will manage post-acute care (PAC) services for our members, replacing eviCore, for any services starting on or after Dec. 1, 2022. These services include home health care, skilled nursing facilities, acute rehabilitation, and long-term acute care hospital admissions. Providers will contact EmblemHealth to help arrange for PAC services and support. Note: eviCore will continue to handle durable medical equipment for EmblemHealth members and manage ongoing care requests for PAC services that began prior to Dec. 1, 2022.
We’re hosting a series of informational webinars to help explain how EmblemHealth’s management of all PAC services and the Transitions of Care (TOC) program affect the members in your care. We invite you to join the session that’s best suited for your facility/provider type – no registration required. All sessions will be held via Microsoft Teams between Nov. 8 and Nov. 17, 2022. See session dates and times.
Attention home health care providers: Our provider portal has a Home Health Status report to give you a combined view of the preauthorization requests for all members under your care. See the quick guide and video in the Preauthorization section of our Portal Training page for instructions on how to create the Home Health Status report.
A new Home Health Care Services Reimbursement Policy is being introduced in support of the PAC insourcing noted above.
Additional support: We offer a Transitions of Care (TOC) program to help members with complex care issues better manage their health after a hospital admission. Our team will work with the member to make sure they are educated about their condition, have needed resources, and get the right care to feel better. Program information is available to our members on the Live Well section of our member website.

Checking a Provider’s Network Status Just Got Easier
We have added the new Check Provider Network Status feature to the Member Management menu of the provider portal. Now, you can also search for a specific provider using your Tax ID, an improvement we added to the current name or NPI search selections. You will be shown a list of affiliated providers to choose from and can then select the provider to see their network status for the selected member.
More Information About Member PCP Assignment
We have enhanced the primary care provider (PCP) history shown on the Member Details page when checking a member’s eligibility or your provider network status. The updated screen shows the Medical Center details so you can see to which practice the member was previously assigned. We hope this information will help you with coordination of care when a member changes PCPs.
Improved PCP Panel Reports
You can now run a single PCP Panel Report for all PCPs (or a subset) who fall under the same Tax ID. The report now has columns showing the currently assigned PCP: provider name, NPI, and address.
Enhanced Claims Reports – More Service Line Details
For medical and hospital claims, the downloadable Excel report now captures the following information from the Explanation of Payment:
- Check paid date (“Paid date”)
- Check number (“Check/Transaction ID”)
- “Payment Reference ID”
- Payee name (“Payee”)
For dental claims, the downloadable Excel report now captures the following:
- Service Line Information
- Area of Oral Cavity
- Tooth
- Surface
- Remittance Information
- Check paid date (“Paid date”)

Dec. 1 is World AIDS Day
EmblemHealth and ConnectiCare honor World AIDS Day Dec. 1 by focusing on prevention and offering you support in caring for our members who have HIV/AIDS.
HIV/AIDS Prevention – PrEP
HIV PrEP is a preexposure prophylaxis (PrEP). It may protect patients against HIV, even if their partner has HIV. Here are a few resources for you and your patients:

We ask our New York providers to consider registering for inclusion in the New York State Department of Health PrEP Prescribers directory.
Our HIV/AIDS Care Management Programs and Resources
EmblemHealth has a free, dedicated HIV/AIDS Care Management program to help members find community resources and navigate the health care system. Members can enroll by calling us at 800-447-0768 (TTY: 711), Monday through Friday, 9 a.m. to 5 p.m. You can also refer your members to these HIV/AIDS Resources. For more information about caring for members with HIV/AIDS, see the New York State Department of Health (NYSDOH) AIDS Institute.
ConnectiCare offers members with HIV/AIDS support through our Care Management program. To refer a member, call 800-390-3522 Monday, Thursday, and Friday from 8 a.m. to 4 p.m., or Tuesday and Wednesday from 8 a.m. to 7:30 p.m.
Mental Health and the Holiday Season
Many people look forward to the holiday season and enjoy the festivities with family and friends. But for others, this time of year can lead to feelings of sadness and trigger increased stress, anxiety, and depression. When caring for our members, be sure to look out for signs of mental illness and, if needed, refer patients for behavioral health services.

Hello, colleagues.
I am Dr. Lama El Zein, senior medical director for population health and clinician engagement at EmblemHealth. I am also a practicing family physician at AdvantageCare Physicians (ACPNY).* My focus is on care management, creating equitable programs around chronic and advanced illnesses, and value-based care initiatives.
A Unique Perspective on Population Health
One of our key goals is to help create healthier futures for our members. To help us meet this goal and influence better health outcomes, we are adopting a population health approach. As doctors working as medical directors for EmblemHealth and ConnectiCare, we are uniquely situated to influence population health. Having you as our partners – supported by data and technology – gives us a broader perspective than what we would see from just treating patients in our own medical practices.
Changing Ourselves First
In adopting a population health approach, the first thing we are changing is ourselves as medical directors. We are redesigning our role from a traditional, reactive, utilization management-focus to a proactive, multifaceted leadership-focus. We are acquiring new skill sets and have staffed our team with diverse personality traits, cultures, and backgrounds. This diversity gives us better insights into those we serve and makes us better partners for you.
Transitions of Care
Our data helps us uncover opportunities to improve transitions of care across our full range of services. We are in the final stages of transitioning the management of our post-acute care programs from outside relationships to our own teams. This will allow us to build our clinical partnerships with you, optimizing the transition of care experience when a member is discharged home from a hospital, skilled nursing facility, or other inpatient site. We appreciate our hospital partners who offer our members virtual visits post-discharge and are here to support post-discharge visits to reduce readmissions.

Do Not Bill Members with Full Medicaid or QMB
If Medicare/Medicaid dual-eligible individuals have their Part A and Part B cost-share fully covered by their Medicaid plan or if they are Qualified Medicare Beneficiaries (QMB), they are not responsible for their Medicare Advantage cost-share for covered services. Do not balance bill these members for any other costs. Any Medicare and Medicaid payments for services given to these members must be accepted as payment in full.
For EmblemHealth members, you can use ePaces to check whether the member has full or partial Medicaid benefits. For more detail, see EmblemHealth Medicare Advantage Plans.
For ConnectiCare members, you can visit CT Department of Social Services or call 800-842-8440. For more detail, see ConnectiCare Medicare Advantage Plans.
Medicare Outpatient Observation Notice (MOON)
All hospitals and critical access hospitals are required by CMS to provide Medicare beneficiaries, including Medicare Advantage enrollees, with the Office of Management and Budget (OMB)-approved Medicare Outpatient Observation Notice (MOON). The MOON and instructions for completing it are available on CMS’ website.
NY Medicaid, HARP, and Child Health Plus Updates
Expansion of Child Health Plus Covered Health Services for 2023
Effective Jan. 1, 2023, EmblemHealth will expand Child Health Plus (CHPlus) covered health services.
Medicaid Perinatal Care Standards
New York State (NYS) issued updated policies on Medicaid Perinatal Care Standards. The updates, effective Oct. 1, 2022, apply to EmblemHealth-contracted perinatal care providers who offer prenatal/antepartum care, intrapartum care, and/or postpartum care to Medicaid and HARP members.
Change of Address (and Contact) Notification
Providers must notify Medicaid of any change of address, telephone number, or other pertinent information within 15 days of the change. For more information on this requirement and how to submit changes, see Reminder: Keep Your Directory Data Current.

EmblemHealth Essential Plan Awarded Tier 1 Quality Incentive
EmblemHealth’s Essential Plan was recently awarded Tier 1 from the Quality Incentive Program in New York State, the highest tier based on the State’s set of quality points. The award recognized the plan’s performance in preventive care, mental health measures, and member satisfaction, along with bonus points achieved through our development of a Health Disparities Implementation Plan. We thank you for your efforts to give our members the care they need. It is clearly making a difference!

Claims Corner
Avalon Laboratory Management Program
Lab testing is highly complex and constantly evolving. Starting March 1, 2023, EmblemHealth and ConnectiCare will begin a new collaboration with Avalon Healthcare Solutions (Avalon) to implement a Laboratory Benefit Management (LBM) program. This will help us stay current and process laboratory claims with greater accuracy and consistency.
Using their expertise in laboratory services, Avalon will review laboratory services performed in office, hospital outpatient, and independent laboratory settings reported on claims (post-service and pre-payment) for adherence and consistency with our laboratory policies and guidelines, as well as industry standardized rules. Avalon will provide routine testing management services with automated review of high-volume, low-cost laboratory tests.
In support of this program, we will be introducing a new payment integrity policy as of March 1, 2023, as well as new and revised reimbursement policies and guidelines.
The new Laboratory Benefit Management Payment Integrity Policy details the program components, including impacted laboratory services, tests, and procedures. It also includes a section of anticipated frequently asked questions (FAQs).
To provide transparency regarding this program and the review standards that will be used, these reimbursement policies and guidelines will be added to the following sections of our websites in the coming weeks for your review:

Reimbursement Policy Updates
New Policies
Starting Dec. 1, 2022, in support of the EmblemHealth PAC insourcing initiative discussed above , we are introducing a new Home Health Care Services Reimbursement Policy for both EmblemHealth and ConnectiCare.
Starting Jan. 1, 2023, a new From/To Date Span Policy, Facility and Professional
(Commercial, Medicare, and Medicaid) Reimbursement Policy is being implemented to ensure we are only paying claims with properly documented dates of service. If the date of service submitted for a procedure is out of the “From” and “To” date calendar range or not in the correct format, claims will be denied.

Preauthorization Lists Revised with CMS’ Quarterly Update for October 2022
EmblemHealth and ConnectiCare have updated all preauthorization lists to reflect the code changes shared in CMS’ quarterly update for October 2022. To see which codes require preauthorization, we recommend using the Preauthorization Check Tool in the provider portals. You may also see the preauthorization lists on our websites (EmblemHealth and ConnectiCare). In the event of a discrepancy, the published list will prevail.

National Influenza Vaccination Week
The Centers for Disease Control and Prevention (CDC) National Influenza Vaccination Week is Dec. 5 to 11. According to the CDC, National Influenza Vaccination Week is a call to all Americans 6 months and older to get their annual flu vaccine. Flu remains a significant public health concern. There is still time to get a flu vaccine to help prevent flu illness and potentially serious complications.

If a provider in your practice is leaving, kindly inform us as soon as possible. To report other changes, sign in to your Provider/Practice Profile for EmblemHealth or ConnectiCare. If you participate with us under a delegated credentialing agreement, have your administrator submit these changes. See more on how to submit changes for EmblemHealth and ConnectiCare.
Remember to review your CAQH application every 120 days and ensure you have authorized EmblemHealth as an eligible plan to access your CAQH information.
