New In-House Post-Acute Care Process Coming
Before the end of 2022, EmblemHealth will begin to manage post-acute care (PAC), replacing eviCore, for EmblemHealth members. This includes home health care, skilled nursing facilities, acute rehabilitation, and long-term acute care hospital admissions.
Once we announce the start date, you will need to contact EmblemHealth to help arrange for PAC services and support. Note: eviCore will continue to handle durable medical equipment for EmblemHealth members.
Upcoming webinars: To help you better understand how the insourcing of all PAC services and the Transitions of Care program affects the members in your care, we will host a series of informational webinars. Session dates and times will be announced in the coming weeks and held via Microsoft Teams. Plan to join the session that’s best suited for your facility/provider type. No registration will be required.
Attention home health care providers: Our provider portal has a Home Health Status report to give you a consolidated view of the preauthorization requests for the members under your care. See the quick guide and video on our Portal Training page in the Preauthorization section for instructions on how to create the Home Health Status report.
Additional support: We offer a Transitions of Care program to help members with complex care issues better manage their health after a hospital admission. Our team will work with the member to make sure they are educated about their condition, have needed resources, and get the right care to feel better. Program information is available to our members on the Live Well section of our member website.
New on the Provider Portal: Check Network Status
NEW: Check Provider Network Status
We offer a wide variety of networks and benefit plans for our members to choose from. We heard your frustration in determining whether you or a provider you manage* is in-network for a specific member – especially when the member has access to multiple networks through the Bridge Program or has reciprocal network access between EmblemHealth and ConnectiCare.
To help you, your Appointment Schedulers, and Insurance Verifiers determine if the selected provider* is in-network for a specific member, we added an easy, time-saving lookup tool on the provider portal’s Member Details page.
Now when you get to the Member Details page, you will see a new Check Provider Network Status button in the main section at the top of the page. After clicking the button, the member’s information will carry forward to a new screen where you can search for the provider in your practice* the member wants to see. The search results will display the provider’s network status for that member in the right-most column in the results table.
As with all member details shared on the portal, being in the member’s network is not a guarantee of payment. Payment is subject to the member’s participation, covered benefits, Managing Entity assignment, referrals, preauthorizations, etc., on the date(s) of service, as applicable.
*Portal user must be authorized to conduct business for the provider they are looking up.

We moved the member ID card section to the top to make it easier for you to find. Expand the section to see an image of the card.

New CDC Recommendations for Flu Vaccines
Encourage your patients to protect themselves by getting a flu shot. The CDC recommends that people get vaccinated against the flu by the end of October, although getting one after can still provide protection during peak flu season.
Annual flu vaccination has been recommended since 2010 for everyone 6 months and older, with few exceptions. New from the CDC this season is a preferential recommendation of a higher dose and adjuvanted flu vaccines in people 65 years of age and older.
For the 2022-2023 flu season, there are three flu vaccines that are preferentially recommended for people 65 years and older:
- Fluzone High-Dose Quadrivalent vaccine
- Flublok Quadrivalent recombinant flu vaccine
- Fluad Quadrivalent adjuvanted flu vaccine

November is Diabetes Awareness Month
November is Diabetes Awareness Month. The Centers for Disease Control and Prevention (CDC) recommends the following preventive practices to help people living with diabetes manage their condition and improve their health:
- Regular A1c testing
- Annual foot exam
- Annual eye exam
- Diabetes self-management education and support (DSMES) classes
- Physical activity
- Not smoking
- Daily blood sugar monitoring

Hello Colleagues,
Fall starts the unofficial eating season with Halloween treats and Thanksgiving feasts, followed by December holiday parties galore! For those with diabetes, these special occasions can be challenging. For those who do not know they have diabetes, these indulgences can take a heavy toll. The issue is so serious that November has a dedicated month (American Diabetes Month), week (National Diabetes Education Week: November 6 – 12), and day (World Diabetes Day: November 14) to call attention to it.
We understand the complexity of living with, and treating, diabetes. We usually recommend an interdisciplinary approach informed by current, evidence-based guidance. Medical societies update their guidelines frequently (see below for the current guidance) and we keep an eye out for their changes to stay informed.
We are here to assist you with the extra help you may need coordinating care and encouraging your patients to practice self-care. For example, when one member needed supplies to check their blood sugar, our Care Manager coordinated with their primary care provider and pharmacy to have $0 copay testing supplies sent directly to the member’s home. The Care Manager then made sure the member had full understanding of what they needed to do to keep their diabetes under control.
We have many resources to support you and your patients. We recently added care plans to our provider portal to give all treating providers easy access to them and to support coordination of care. See below for programs, classes, and materials we have available.
Chronic Obstructive Pulmonary Disease
Diabetes is only one of many complex conditions our members are dealing with. November is also COPD Awareness Month.
Chronic obstructive pulmonary disease (COPD) refers to a group of diseases that cause airflow blockage and breathing-related problems. We want to support you while you take care of our members. Services available for your patients with COPD include dedicated Care Management programs for patients who need disease education and coordination of care.
Vaccines
October is also the time of year for all of us to continue to drive home the message concerning the importance of vaccinations. Visit CDC.gov for vaccine considerations and what health care professionals should know about:
- MVP (formerly referred to as monkeypox)
- Polio
- Influenza
- COVID-19
We recommended vaccinations be considered according to age group. See the Prevention information available to our members.
Programs, Classes, and Resources
- We offer Care/Case Management programs for patients with complicated diabetes with comorbidities such as coronary artery disease (CAD) or chronic kidney disease (CKD), COPD, complex care, and more. Members have access to nurse care managers, social workers, and pharmacists depending on their individual needs. Established care plans are shared on our provider portal—so set up an account if you haven’t already. To refer a member to one of our Care/Case Management programs, you may contact us at:

Medicare
2022 Annual Special Needs Plan Model of Care Training
The Centers for Medicare & Medicaid Services (CMS) requires providers to complete training for each dual-eligible special needs plan (D-SNP) they participate in. Some providers are past due for this training and certification. If you haven’t already done so, you must submit an attestation to receive a certificate (ConnectiCare) or confirmation (EmblemHealth) of completion. Our trainings take only 15 minutes to complete.
ConnectiCare: Providers who may care for ConnectiCare’s Medicare Advantage members with Choice Dual (HMO D-SNP) plans need to complete ConnectiCare’s Special Needs Plan Model of Care (SNP MOC) training.
EmblemHealth: EmblemHealth’s VIP Bold Network and Reserve Network providers must complete the 2022 EmblemHealth SNP MOC annual provider training. We will send instructions for the new simplified process for completing and attesting to the training to eligible providers.

NY Medicaid, HARP, and Child Health Plus Updates
MPV Vaccine Administration
MPV vaccine (formerly known as monkeypox) administration is a covered benefit for EmblemHealth Medicaid members. We will reimburse providers for the administration of these vaccines. For more information on immunizations, mandatory reporting, and testing locations for MPV, visit our provider hub for State-Sponsored Programs.
Change of Address (and Contact) Notification
Providers must notify Medicaid of any change of address, telephone number, or other pertinent information within 15 days of the change. For more information on this requirement and how to submit changes, see Reminder: Keep Your Directory Data Current.

Medicaid Cultural Competency Training
Each year, the New York State Department of Health requires EmblemHealth Enhanced Care (Medicaid Managed Care) and Enhanced Care Plus (HARP) participating providers to certify completion of cultural competency training for all staff who have regular and substantial contact with our members. To satisfy this training requirement, providers and their staff must complete the U.S. Department of Health & Human Services e-learning program, The Guide to Providing Effective Communication and Language Assistance Services and providers must submit the applicable certification. If you and your staff have already completed the training with another managed care plan, you are not required to complete the training again; however, you must certify to us that the training has been completed.
NYSDOH Wage Parity Compliance Deadline Nov. 1
The New York State Department of Health (NYSDOH) issued an update on wage parity compliance and certification guidance. The update extends the submission date for required wage parity compliance activities. For details, read the Annual Home Care Worker Wage Parity Compliance Requirements Update.

Reimbursement Policy Updates
We are introducing a formal Ophthalmology Services Reimbursement Policy for EmblemHealth and ConnectiCare.
We have updated the Commercial Preventive Services grids (EmblemHealth|ConnectiCare). See the Revision History for applicable changes and their respective effective dates.
Effective Jan. 1, 2023, the Coding Edits reimbursement policies will include new edits applicable to modifier JW (drug amount discarded/not administered to any patient) (EmblemHealth|ConnectiCare).
Reminders: Reimbursement Policies Effective Oct. 1, 2022
On Oct. 1, 2022, a new reimbursement policy for both EmblemHealth and ConnectiCare was introduced to give guidance regarding appropriate billing for services provided in a hospital/facility Treatment Room. EmblemHealth and ConnectiCare will reimburse treatment room services when a specific, identifiable procedure has been performed or a treatment rendered that is unrelated to inpatient or outpatient services is provided within the contracted global period. Treatment room services that are rendered for the sole purpose of performing a laboratory, radiology, or other diagnostic test will not be reimbursed.

Free In-Home Screening Kits for Eligible Plan Medicaid and Medicare Members
EmblemHealth and ConnectiCare have partnered with Quest Diagnostics to provide free in-home colorectal cancer and diabetes screening test kits to our eligible Medicaid and Medicare members. This program aims to increase screening adherence and improve health outcomes.
Eligible EmblemHealth Medicaid and Medicare members can receive:
- A fecal occult blood test (FOBT) to screen for colorectal cancer
Eligible ConnectiCare Medicare members can receive:
- FOBT for colorectal cancer
- The A1c test for diabetes

EmblemHealth Preauthorization Updates
Starting Jan. 15, 2023, the EmblemHealth Preauthorization Lists will include four new requirements for genetic/molecular services (HCPCS Codes: 0012M, 0013M, 0014U, and 0017M). For details, see the Notable Changes announcement.

Commercial Updates
REVISED: Benefit Updates for City of New York Members
Last month, we told you of several changes that would impact City of New York GHI CBP Plan members. There have been some minor changes since then. Here is the current guidance:
On Oct. 1, 2022, the following went into effect:
- All Montefiore providers will have a $15 primary care provider (PCP) copay or a $30 specialist copay.
- ProHEALTH Urgent Care centers will have a $100 copay. All other participating urgent care centers will continue to have a $50 copay.
Beginning Nov. 1, 2022, all high-tech radiology imaging procedures received at participating RadNet facilities, Memorial Sloan Kettering Cancer Center, and Hospital for Special Surgery will continue to have a $50 copay. All high-tech radiology services received at other participating facilities in New York State will have a $100 copay ($50 outside of New York State). Services include, but are not limited to: 3DI, CT Scan, MRA, MRI, Nuclear Medicine, and PET Scan.

The following medical policies have been revised for both EmblemHealth and ConnectiCare:
Bariatric Surgery (EmblemHealth|ConnectiCare): Added language communicating that the Overstitch device (endoscopic revision) is considered investigational.
Cardiac Event Monitors (EmblemHealth|ConnectiCare): Added language communicating that CPT 0716T (Acoustic waveform recording with automated analysis and generation of coronary artery disease risk score) is considered investigational.
Noninvasive Prenatal Testing (NIPT) for Fetal Aneuploidy (EmblemHealth|ConnectiCare):
- Expanded Commercial coverage to include twin pregnancies effective Nov. 12, 2022.
- Removed Fetal Rhesus D MCG cross reference
Obstructive Sleep Apnea Diagnosis and Treatment (EmblemHealth|ConnectiCare): Added BMI ≥ 45 as a covered indication for polysomnography.
EmblemHealth’s Gene Expression Profiling:
- Added whole genome/exome sequencing section citing medically necessary MCG criteria sets
- Updated Limitations/Exclusions section citing investigational MCG criteria sets
ConnectiCare’s Infertility (Commercial): Removed language within Donor Services section pertaining to age 40.
One Updated Medical policy – ConnectiCare:
ConnectiCare has updated the Experimental, Investigational or Unproven Services Policy to include additional codes – see revision history for applicable changes/updates with effective dates.
ConnectiCare defines the terms “investigational” or “experimental” as the use of a service, procedure, or supply that is not recognized by the health plan as standard medical care for the condition, disease, illness, or injury being treated. A service, procedure, or supply includes, but is not limited to, the diagnostic service, treatment, facility, equipment, drug, or device.

If a provider in your practice is leaving, please inform us as soon as possible. To report other changes, sign in to your Provider/Practice Profile for EmblemHealth or ConnectiCare. If you participate with us under a delegated credentialing agreement, please have your administrator submit these changes. See more on how to submit changes for EmblemHealth and ConnectiCare.
Remember to review your CAQH application every 120 days and ensure you have authorized EmblemHealth as an eligible plan to access your CAQH information.
