HIV/AIDS Resources
We created a library of flyers describing EmblemHealth’s Case/Care Management programs, including one for the EmblemHealth’s HIV/AIDS Care Management program. Members can enroll by calling us at 800-447-0768 (TTY: 711), Monday through Friday, 9 a.m. to 5 p.m.
ConnectiCare offers members with HIV/AIDS support through our Care Management program. To refer a member, call 800-390-3522 Monday, Thursday, and Friday from 8 a.m. to 4 p.m., or Tuesday and Wednesday from 8 a.m. to 7:30 p.m.

You may also call 988 from any state to obtain free, confidential mental health support 24/7 via the 988 Suicide & Crisis Lifeline. In the case of a life-threatening emergency, please call 911.

You can find health care proxy, do not resuscitate (DNR), or physician orders for life-sustaining treatment (POLST) and MOLST forms at the State Department of Health.

This online tool will guide them through the process of documenting their wishes and preparing the forms.

New Jersey
Programs and Classes
- We offer Care/Case Management programs for patients with complicated diabetes with comorbidities such as coronary artery disease (CAD) or chronic kidney disease (CKD), COPD, complex care, and more. Members have access to nurse care managers, social workers, and pharmacists depending on their individual needs. Established care plans are shared on our provider portal—so set up an account if you haven’t already. To refer a member to one of our Care/Case Management programs, you may contact us at:

- EmblemHealth’s Pharmacy Programs can help with comprehensive medication reviews and we offer a Medicare-specific Medication Therapy Management Program to support adherence. We strive to integrate the latest pharmacological updates to prevent diabetes complications.
- Online virtual and in-person classes are available at our EmblemHealth Neighborhood Care and ConnectiCare Centers.
