Starting July 15, 2024, we will be updating our Site of Service Medical Policy – Infusions and Injectables to add 170 medications and remove the age restrictions so the policy will apply to all commercial members, not just those over the age of 21. We are modifying our protocols to ensure that injectable and infusion medications are administered at the most clinically appropriate site of care.
These drugs already require preauthorization. However, they may no longer be covered when administered in a hospital setting and may require administration in a member’s home or other outpatient location. We are making this change to expand the successes we are seeing in lowering member’s out of pocket costs and providing convenient access to care for ongoing maintenance infusion services. The 2024 Quality Incentive Program is underway for eligible EmblemHealth and ConnectiCare providers. A new brochure highlights this provider network program to show how eligible providers can be rewarded for delivering better health outcomes. Home Care Extensions
We have good news for our Home Care Agencies!
Caring for patients who are recovering from surgeries and illnesses means that the predicted care needs don’t always match the patients’ actual needs. We understand that you need a way to request an extension if you discover a patient needs more assistance than allotted for in the preauthorization.
We heard your concerns. Based on your feedback, we updated our systems so they won’t reject your preauthorization requests for extensions.
Please start using the provider portal now for all new extension requests.
Reminder: Fax Lines To Be Disconnected May 2024
Starting May 1, 2024, we will disconnect the fax lines currently in use for preauthorization requests and concurrent reviews. Please use the provider portal to submit your requests and supporting documentation. See original announcements (EmblemHealth | ConnectiCare) sent in December 2023. We also recently sent an email.
Serving our members requires knowing their needs, including the languages they speak. We serve a diverse membership whose top languages include English, Spanish, Chinese, and Italian. As visuals often send a powerful message, we created maps of our EmblemHealth and ConnectiCare core service areas to get a county-level view of these languages. We encourage you to look for your county and see how our members communicate. Data used for these maps is representative of active membership at the end of January 2024.
EmblemHealth and ConnectiCare partner with Episource to conduct Medicare and NY State of Health (NYSOH)/Access Health CT chart reviews required by the Centers for Medicare & Medicaid Services (CMS).
The purpose of the chart review is to capture proper ICD-10 coding and identify any areas of improvement. Instances of improper coding will be identified during the review and shared with you. Proper coding helps us better serve our members.
If you receive a medical record request from Episource, please follow the instructions and send the requested documentation directly to Episource as required by your participating provider agreement. Medical record requests will occur throughout the year. We appreciate your prompt response to all Episource medical record requests.

In observance of Maternal Mental Health Week (April 29 to May 5), I focus on the importance of screening for adults who may have postpartum depression (PPD) as well as mention a new prescription medication for the treatment of adults with PPD. See full blog.

The mailbox should not be used for patient-specific issues, claims inquiries, complaints, grievances, appeals, etc., and should not include protected health information (PHI). For those purposes, use the Claim Inquiry and Message Center features of the provider portals (EmblemHealth | ConnectiCare).
All hospitals and critical access hospitals are required by CMS to provide Medicare beneficiaries, including Medicare Advantage enrollees, with the Office of Management and Budget-approved Medicare Outpatient Observation Notice (MOON). The MOON and instructions for completing it are available on the CMS website.
UPDATE: Effective Jan. 1, 2024, EmblemHealth expanded the populations eligible for Community Health Worker (CHW) services beyond pregnant and postpartum populations. CHW services are now available to all children under 21 years of age, adults with chronic conditions, justice-involved individuals, those with unmet health-related social care needs, and individuals experiencing community violence.
Providers must notify Medicaid of any change of address, telephone number, or other pertinent information within 15 days of the change. For more information on this requirement and how to submit changes, see Reminder: Keep Your Directory Data Current.

ConnectiCare has updated its Billing and Claims guidance to clarify that the 18-month claims adjustment look-back period for commercial and Medicare members applies to underpayments as well as overpayments and refunds.
As announced in the March 2024 edition of Office Visit, effective June 15, 2024, we are introducing a new Compression Garments reimbursement policy for EmblemHealth and ConnectiCare. EmblemHealth and ConnectiCare will follow CMS reimbursement guidelines for compression garments/stockings when prescribed by a doctor and provided by a durable medical equipment (DME) supplier. We have updated the policy to reflect the new codes introduced as part of the quarterly update for January 2024. See the Revision History for details.
National Women’s Health Week begins on Mother’s Day each year to encourage women and girls to make their health a priority. Women represent the cornerstone of a family's overall health. Ensuring they have access to quality care can also lead to improved health for children and families.
According to the CDC, two out of every three caregivers in the United States are women. This means they provide daily or regular support to children, adults, or people with chronic illnesses or disabilities. Women who are caregivers have a greater risk for poor physical and mental health.

EmblemHealth in-network cardiologists can take part in this program and make immediate appointments for referred patients by calling 631-396-8650 from 8 a.m. to 6 p.m., Monday through Thursday and from 8 a.m. to 5 p.m., Friday. Preauthorization is not required and details of your patient’s visit are shared with you.

Let Us Know When Directory Information Changes
If a provider in your practice is leaving, please inform us as soon as possible. To report other changes, sign in to your Provider/Practice Profile for EmblemHealth or ConnectiCare. If you participate with us under a delegated credentialing agreement, please have your administrator submit these changes. See more on how to submit changes for EmblemHealth and ConnectiCare.
Remember to review your CAQH application every 120 days and ensure you have authorized EmblemHealth as an eligible plan to access your CAQH information.
