An Outpatient Option for Infusion Services: New York Cancer & Blood Specialists (NYCBS)
New York Cancer & Blood Specialists (NYCBS), one of several freestanding infusion centers in the EmblemHealth network, has launched a concierge program to offer EmblemHealth patients a homey, convenient environment for their infusion treatments. Undergoing regular infusion treatments can be distressing for patients. NYCBS’s clean, quiet, comfortable, and spacious suites can help alleviate patient anxiety. Referring providers will also have the benefit of working with NYCBS’s dedicated staff who will keep them informed of their patients’ treatment.

If your practice or your patients are not located near an NYCBS location, know EmblemHealth has contracts with Option Care Health and Coram, too. In Connecticut, members can go to Fresenius. Provider Portal Updates
Enhanced Printing Capabilities
We are enhancing our portals’ printing functionality to offer a consistent and complete presentation of information. The areas with the most noticeable changes are:
- Preauthorization Requests (medical and dental)
- ER Notifications
- Claims Details
The new print versions will have the same standardized formats for every stage of a transaction. Plus, they will capture all the data elements accessible from the screen and, unlike a screenshot, you can print everything on that screen even if it isn’t visible on the screen.
Protecting Patient Privacy – Supported Browsers Only
We are all responsible for protecting patient health information (PHI). One of the ways we protect PHI is by ensuring that our portals are only accessed by systems with updated, secure internet browsers. As of April, we blocked portal access from computers with outdated browsers. We contacted, and offered assistance to, affected users so they wouldn’t lose access to the portal.
What can you do for an optimal portal experience? We recommend that you use either Google Chrome or Microsoft Edge. No matter which secure browser you use, either of these or another, be sure to regularly update your browser to help keep your information, and our members’ information, secure. Mental Health, Alzheimer’s Disease, and Brain Awareness
May is Mental Health Awareness Month and June is Alzheimer’s and Brain Awareness Month. Since brain health is sometimes affected by age-related changes in the brain, mood disorders (depression), or diseases such as Alzheimer’s disease, we encourage you to talk about brain health with your older patients. Here are some suggested questions to consider:
- How is their balance? (motor function)
- How do they respond to emotions? (emotional function)
- How do they feel and respond to pressure, pain, and temperature? (tactile function)
- How well do they think, learn, and remember? (overall cognitive health)
Encourage your patients to practice a few of the tips below to help prevent Alzheimer’s disease and dementia. Daily doses of the following can improve your patients’ brain health for years to come:
- Maintain an active mind: Take or teach a class, read books, play games, cultivate an old hobby.
- Manage their high blood pressure by encouraging them to monitor it at home and to be sure to get regular provider check-ups.
- Stay connected with social activities: Volunteer, visit friends and family, join a walking group.
- Manage stress: Stay positive, exercise regularly, practice relaxation techniques.

National HIV Testing Day: June 27
According to the CDC, about 80% of HIV infections that occur each year are transmitted by people who are not receiving HIV care. As National HIV Testing Day approaches on June 27, this is a good time to encourage your patients to be tested for HIV, the virus that causes AIDS.
The CDC recommends treating people with HIV rapidly and effectively to reach sustained viral suppression. Patients should begin treatment as soon as possible after receiving a positive diagnosis and take medication as prescribed to achieve and maintain an undetectable viral load. An undetectable viral load is the best thing people with HIV can do to stay healthy.
If you need help managing our members’ needs, you can enroll your them in our Care Management program by calling 800-447-0768 Monday through Friday, 9 a.m. to 5 p.m. Members can find more information about living with HIV on our website. This program assists members in finding community resources and navigating the health care system. There are many CDC resources available for newly diagnosed patients and those who need to reengage in treatment.

2022 EmblemHealth Medicare Member Incentive Program
EmblemHealth will continue to offer an incentive program this year to encourage our Medicare Advantage (including Special Needs Plan) members to receive preventive care, including recommended screenings. EmblemHealth’s Complex Care Management program can help members schedule needed care.

Combating Fraud, Waste, and Abuse is Everyone’s Responsibility
Every year, billions of dollars are improperly spent because of medical fraud, waste, and abuse1 (FWA). It affects everyone, including you. All Medicare-contracted providers and their employees are required by the Centers for Medicare & Medicaid Services (CMS) to complete FWA training when newly hired, and annually thereafter.
To assist you in meeting this annual training requirement, see Required Medicare Training: Fraud, Waste & Abuse. For more information about FWA, refer to EmblemHealth Provider Manual’s Fraud and Abuse chapter. Detection, correction, and prevention of FWA is everyone’s responsibility.

Do Not Bill Members With Full Medicaid or QMB
If Medicare-Medicaid dual-eligible individuals have their Part A and Part B cost-share fully covered by their Medicaid plan, or are Qualified Medicare Beneficiaries (QMBs), they are not responsible for their Medicare Advantage cost-share for covered services. Do not balance bill these members for any other costs. Any Medicare and Medicaid payments for services given to these members must be accepted as payment in full.
For EmblemHealth members, you can use ePACES to check whether the member has full or partial Medicaid benefits. For more detail, see EmblemHealth Medicare Advantage Plans.

Medicare Outpatient Observation Notice (MOON)
All hospitals and critical access hospitals are required by CMS to provide Medicare beneficiaries, including Medicare Advantage enrollees, with the OMB-approved Medicare Outpatient Observation Notice (MOON). The MOON and instructions for completing it are available on CMS’ website.
NY Medicaid, HARP, and Child Health Plus Updates
July 15 Deadline for Required Medicaid Cultural Competency Training Certification
Each year, the New York State Department of Health requires EmblemHealth Enhanced Care (Medicaid Managed Care) and Enhanced Care Plus (HARP) providers to certify completion of cultural competency training for all staff who have regular and substantial contact with our members. This required certification must be submitted to EmblemHealth by July 15, 2022.
Change of Address (and Other Contact) Notification
Providers must notify Medicaid of any change of address, telephone number, or other pertinent information within 15 days of the change. For more information on this requirement and how to submit changes, see Reminder: Keep Your Directory Data Current.

What You Should Know About COVID-19 Vaccines, Tests, and Treatment
The New York State Department of Health (NYSDOH) has created a useful tip sheet COVID-19 Vaccines, Tests, Treatment, and You to help Medicaid recipients understand what is covered, how to access care, and where to obtain more information. Claims Corner
New Payment Integrity Policies
Beginning Aug. 30, EmblemHealth and ConnectiCare will enhance our existing claims editing program by implementing new coding validation edits. Coding validation edits evaluate the correct use of overriding modifiers such as modifiers 25, 59, XE, XS, XP, and XU. Our goal is to improve the overall accuracy of our claims processing for all lines of business by ensuring the correct modifiers are used. To support of this change, we are introducing a Coding Validation Payment Integrity Policy for both EmblemHealth and ConnectiCare.
Reimbursement Policy Updates
We are introducing two new reimbursement policies that apply to both EmblemHealth and ConnectiCare members:
- The new Readmission Policy (EmblemHealth and ConnectiCare) will go into effective Aug. 30, 2022. The policy governs when a readmission will be treated as a separately payable new admission and when as a continuation of the first inpatient stay.
- The Evaluation & Management Services (E&M) Policy (EmblemHealth and ConnectiCare) will replace our Supplemental E&M Policy. In addition, effective Sept. 1, 2022, EmblemHealth will no longer reimburse consultation codes 99241-99255 for our GHI PPO City of New York Plans. Non-consultative E&M codes may be utilized based on the code that best describes the service performed.
The following reimbursement policies have been updated:
- ConnectiCare updated the ASC Grouper Policy to include new codes effective April 1, 2022. Ambulatory surgical groupers will be paid according to surgical contracted rates when billed with revenue codes 360 or 490. If surgical services are billed with revenue codes other than 360 or 490 and the claims contain charges for anesthesia and/or recovery room, claims will be paid according to the surgical contracted rates unless otherwise negotiated.
- EmblemHealth’s Coding Edits Policy was updated to include the additional edits that will be implemented Aug. 1, 2022.
- The COVID-19 Vaccine and Monoclonal Antibody Infusions Policy, which applies to both EmblemHealth and ConnectiCare, was updated to add new codes (Q0220, Q0221, Q0222, M0220, M0221, M0222, M0223, and M0240).
- ConnectiCare has updated the Experimental, Investigational or Unproven Services Policy to include new codes effective April 1, 2022. ConnectiCare defines the terms “investigational” or “experimental” as the use of a service, procedure, or supply that is not recognized by the health plan as standard medical care for the condition, disease, illness, or injury being treated. A service, procedure, or supply includes, but is not limited to, the diagnostic service, treatment, facility, equipment, drug, or device.
Reminder: Telehealth/Virtual Care Services Reimbursement Policies Effective June 1, 2022
As announced in February, we are introducing a new reimbursement policy for Telehealth/Virtual Care Services (EmblemHealth and ConnectiCare) effective June 1. We ask our providers to use telehealth visits, wherever possible, to close gaps in care, meet Healthcare Effectiveness Data and Information Set (HEDIS®) measures, increase Consumer Assessment of Healthcare Providers and Systems (CAHPS®) scores, and improve risk score accuracy.
Note: For EmblemHealth and ConnectiCare Medicare Advantage plans, the enforcement of the Place of Service (POS) requirements (limiting reimbursement to POS-02 Telehealth Provided Other than in Patient’s Home and POS-10 Telehealth Provided in Patient’s Home) will go into effect at the end of the public health emergency (PHE). We ask providers to use the telehealth modifiers listed in the policies to facilitate correct payment.

Starting Aug. 1: New Commercial Plan Site of Service Coverage Rules for Maintenance Infusions
Starting Aug. 1, in support of our commitment to keeping members’ costs down, we are changing how we cover ongoing maintenance infusion services for adults (21+) in our commercial plans for both EmblemHealth and ConnectiCare. Routine maintenance will be covered in the following outpatient settings:
- Home (POS 12)
- Office (POS 11)
- Ambulatory infusion suites (POS 24, POS 49, or POS 12 with Modifier SS)
Exceptions for initial doses and members with justifiable needs to continue care in a hospital outpatient setting will be allowed. Your patients’ current authorizations from Express Scripts’ company Care Continuum will be valid through the date noted in their approval letter.

Preauthorization Lists Updated with CMS’s Quarterly Update for April 2022
EmblemHealth and ConnectiCare have updated their preauthorization lists to reflect the code changes shared in CMS’ quarterly update for April 2022. To see which codes require preauthorization, we recommend using the Preauthorization Check Tool in the provider portals. You may also see the preauthorization lists on our websites (EmblemHealth and ConnectiCare). In the event of a discrepancy, the published list will prevail.

If a provider in your practice is leaving, inform us as soon as possible. To report other changes, sign in to your Provider/Practice Profile for EmblemHealth or ConnectiCare. If you participate with us under a delegated credentialing agreement, have your administrator submit these changes. See more on how to submit changes for EmblemHealth and ConnectiCare.
Remember to review your CAQH application every 120 days and ensure you have authorized EmblemHealth as an eligible plan to access your CAQH information.
